I traveled with Brookledge from New York to Calgary, Alberta and I was very impressed with their horse care and the traveling conditions of the horses. They are a very reputable company with many trucks.
Johnson is very good. Also, I just had someone use Equine Express to ship their new horse from VA to CA, and it seemed to be a good experience. She purchased a 1.5 stall, but they shipped her in a box stall the second leg of the trip.
I shipped my filly from Florida to AZ with Equine Express. I couldn't find anything but positive reviews of them, and at 2 1/2 she stepped off that trailer completely calm (though confused about the mountains in the distance) and as if she had just left her stall in the barn she grew up in.
There are definitely other good shippers as well, but my absolute requirements for any long distance would be air ride trailers the whole way, box stall, and horsepeople as the drivers.
My horse is a dressage diva so I don't have to be.

I used Lorraine to ship a horse from Miami to Maryland last summer and the office staff was dead rude. Brookledge called me back after I'd already given my deposit to Lorraine (I needed to move the horse relatively quickly from Calder) and actually came in cheaper.
Life would be infinitely better if pinatas suddenly appeared throughout the day.
Used Cox to ship my mare from Arizona to Va. Was extremely pleased with the service, the quote and her condition. Have used Johnson to ship from Va. to Fla and equally pleased with them. (Cox ships east to west or vice versa, but not north to south.)
