Default Protocol for using both Adequan and Legend

    Hello COTHers:

My horse came up with a stifle issue in Nov. Big name vet recommended Legend IV x3/ once a week. Local vet says she often uses them in conjunction. Adequan subQ every 2-3 days, followed by 1 injection of Legend. Then drop the Legend and continue with Adequan.

I read on a recent thread that the current reccomendation for Adequan is loading dose, off for 6 mos. Repeat loading dose, no proven effect for continuing Adequan.

Who uses both, what is your protocol, and what do the studies say?

    No pentosan.

    TIA,
    PKN

I have elderly horses with various injuries/arthritis. I use Legend 1x/month and Adequan 1x/week. You could probably do the loading dose of Adequan, then use it and Legend once/month.

Granted, mine are extreme cases, but I've always had better luck with continuous use of Adequan, rather than stop and restart.

I don't use either anymore because I like Pentosan better but when I did use it I would give the loading dose of Adequan ie every four days for 7 doses then give Legend 24 hours out before the race.

The only Adequan protocol that was researched and found to have had disease-modifying effects is the 1 vial every 7 days for 28 days protocol. Other dosing schedules that people have come up with have no proven benefit.
I can't comment on Legend dosing, as I've never seen them used in combination.

When I used both, I used them each once a month. I cannot recall if I gave both on the first or if I gave one on the first and one on the 15th....it has been several years.

I eventually retired the horse because we just could not keep her sound.

I always used them together. Generally I used Legend on week one and adequan 2 weeks later. So horses were getting something every two weeks. I didn't do loading doses. I did still do join injections as needed and if they were showing or the footing was hard I gave legend and adequan the week of the show together. Mine were always quite comfortable. Quite expensive but comfortable.

I hadn't heard about using them together, my question would be... is your horse so severely lame that Adequan, using the loading dose then for maintenance, wouldn't work as well? The vet may have a very good reason, it may be that for certain conditions vets have found that Adequan alone just isn't very effective, but I think its ok to ask if trying Adequan by itself first would still be worth it to keep the cost down a bit.

I have a 26 y/o QH with SEVERE ringbone (and probably arthritis in other joints too). He got 5ml Adequan once every 4 days for 7 doses as a loading dose and gets it monthly for maintenance. He went from being lame all the time (despite joint supplements, therapeutic shoeing, 24/7 turnout to keep him moving, although I'm sure those things help) to almost completely sound after the loading dose plus the next months maintenance dose.

Technically, the loading dose was Chondroprotec. I know, I know, there is no generic adequan and from what I can tell it really varies from horse to horse, some won't respond to Chondroprotec or ICHON but will respond to Adequan. My horse responded wonderfully to Chondroprotec. It happens to be cheaper than Adequan which is a happy bonus.

Sure - lots of the upper level horses in my barn are on both. Vet recommended is loading dose of Adequan spring and fall. Legend shot every 3-4 weeks. Several get an extra Legend 24 hours out from showing.

At one point, I did three doses of Legend one week apart (one dose a week for three weeks), followed by an alternating schedule of Legend and Adequan two weeks apart (so, he got Legend once a month, and Adequan once a month on a schedule where he was getting one or the other every two weeks - does that make sense?).

I didn't have any "problems" with it, but it didn't really help all that much. But I think that is because the horse actually had EPSM instead of a stifle issue.

Per my vet I used adequan once a month and legend once a month seperated by 2 weeks, so every two weeks he got one or the other. It absolutely helped him through the busy season...

Look at the drug pharmacology, the place to start is always with the researched, proven protocols - if they aren't effective for your horse & you've exhausted all alternate drugs, then start experimenting ...

You may be certain that the manufacturers tested many permutations before releasing a drug to market, lameness specialists that treat 1000's of horses, that can offer details behind modifications to the recommended protocols would be my basis for choosing altered drug therapy - much more efficient than trialing intuitive drug therapies.
