Sad to say, I had an Aussie that had similar, though less intense problems. I, too, got him as a puppy and everything seemed good for about the first two years. He didnt seem abnormally shy, but he did have issues with other dogs at obedience - somwhat aggressive towards the larger dogs. As he got older he developed severe separation anxiety - tried to chew though a window and nearly suceded! Tried Clomicalm with no help. Ace helped some but the best was staying in his crate in his preferred room. He also was unrelieble with people. Nipped my Uncle when he sneezed and my Dad when he walked by. Fortunately his bites were always inhibited nips. Dodman only suggested more medication. He didnt do the meltdown you describe though.
Essentially, I just managed his environment. As much exercise as possible and not trusting him around other people. Eventually he went blind and did not handle it well with his anxiety. He was euth'd just shy of 13 years old. I often wondered about his brain function or possibly a tumor especially after he went blind - didnt matter really as I dont know if we could have done anything different. With yours showing seizure-type activity, I wonder if your vet would be willing to try anti-seizure medication to see if that improves things?

Has your vet run a blood panel for allergies? It is my understanding that undiagnosed allergies can sometimes cause anxiety. We've had a lot of success with the Spectrum Spot Test: http://www.vetallergy.com/pages/testingpanels.html

You may have mentioned this and I missed it, was the dog checked for diabetes?

I just stumbled over this link to a site about dogs with fear issues, not sure if it will help or not.

Yes, I will be reading it for a while, and see what I can pick up in training information for my barnyard stray since she has fear issues.
