I'd find a vet dermatologist in your area and take your dog for skin allergy testing. There are allergy tests that use a few tubes of blood, but they are not anywhere near as accurate as a skin test....

Walgreens or CVS sell small, red sharps containers. You'll find them along with diabetic supplies (alcohol swabs, glucose tablets). One has lasted me a few years; works great for needles.
