When you pinch her skin, does it go back down right away or does it stay kind of pinched away from her body? You can also test her gums to see if she's dehydrated.
What kind of trough is it? My filly climbs into the pasture's black Rubbermaid trough pretty regularly and never gets hurt. It's not near any fencing though either.

We had a bad blizzard last year, and my mares were in box stalls with exactly zero turnout for 3-4 days. They got to run around loose in the indoor for maybe 20-30 minutes each day, that was it.
My mare has the same issue - I try to find blankets/sheets with velcro or zip-off tail flaps. No tail flap, no problems!
I was actually surfing both boards at once and noticed it. What OP is doing is pretty much abuse in my book, and I think that should be brought to others' attention.
