It looks like the lad is off to the side and examining Gimcrack's rear hock. (http://resources3.news.com.au/images/2011/04/07/1226035/139143-a-christie-039-s-employee-shows-039-gimcrack-on-newmarket-heath-039-by-george-stubbs.jpg) I posted the store elsewhere with a larger version of the image.
The painting is 6' wide and actually comprises two scenes: one showing Gimcrack winning to the right, and outside his stable on left with jockey, trainer and stable-lad.
Gimcrack was a tough little horse (14hh) who was noted as very popular with the racing public of the time.
