USEA rider achievement

Is the USEA still doing the USEA Rider Achievement Awards and Volunteer Awards? I was looking on the site and there is reference made to the 2009 year. Yet I can not find the forms to fill out for the awards, only forms I am finding are for the medals. Any links would be great if you find them;)
